The transmitral Doppler shows an E wave exceeding the A wave in velocity. This finding, by itself, in a patient with heart disease, is not very helpful, as it can represent either normal or “pseudonormal” filling. The former is associated with normal filling pressures, the latter with elevated filling pressures. Therefore, more data are necessary to evaluate filling pressures. These data are usually furnished by inspecting the pulmonary vein spectral Doppler, tissue Doppler of the mitral annulus, or, ideally, both. The duration of the transmitral A wave is 130 msec (Figure 7); the duration of the pulmonary vein diastolic A reversal velocity is roughly 200 msec (Figure 8); the A reversal velocity is high (roughly 50 cm/sec). When the pulmonary venous A duration exceeds the mitral A duration by more than 50 msec, it is indicative of an elevation of the LV end-diastolic pressure exceeding 15 mm Hg (see Figure 234 in the Echocardiography Syllabus).To obtain an estimate of LA pressure, one should inspect the transmitral E velocity and the tissue Doppler E’. These values are approximately 120 and 9 cm/sec. The quotient E/E’ is therefore 13, which is an intermediate value (between 10 and 15), suggestive of mild elevations in LA pressure. While LA and LV diastolic pressures do track together, there are instances where the mean LA (or pulmonary capillary wedge pressure) can be lower than the post-A wave LV end-diastolic pressure. This may be seen in instances where the LV has very poor compliance (e.g., coronary heart or hypertensive heart disease) and there is a large increment in pressure following atrial contraction. Such a patient might be expected to fare poorly with the onset of atrial fibrillation, as this “booster function” may serve to preserve ventricular fiber stretch despite only modest elevation in LA pressure. Therefore, of the choices given, option C comes closest to describing the clinical findings.

HBO Copy Protection

The only way around HBO copy protection I have been able to find is to just bypass Media Center altogether and use the "capture video" feature in Sonic MyDVD for an mpeg file that will burn to a DVD (two hour-long shows per disk) and play on any player. For a dvr.ms file that refuses to burn directly in Media Center (copy protection again) use Windows Explorer to find the file, then right-click and select 'burn to DVD'. This defaults to Sonic Record Now, and for some reason seems to bypass whatever makes Media Center (or Sonic MyDVD) refuse to open the file. I get a DVD that will only play on the Media Center computer, but at least it enables me to store movies I want to keep on removable media rather than the hard drive. This only works for movies 2 hours or less in length.

Hey break Iraq into two countires; in the north, Kurdistan and in the center and south just turn Iraq back over to some faction of the sunis that we can live with and who are strong/brutal enough to form an effective government. They have always been natural enemies of Iran and would be again. Iran is playing these games because we have basically wiped out all of thier natural enemies, ie Afganistan,Iraq.When you create power vacuums they invariably get filled. Our foriegn policy is so ideallistic and simplistic that its good thing we have a great military to protect us. Good intentions (ours) have killed a lot of people for stupid reasons.

Dear all,

Need it be said.. It is HOT outside! After worship services and lunch I decided to walk..or should I say, stagger for 40 minutes..it seemed like an hour and forty minutes . I heard rolling thunder above me in the sky but not a drop of rain hit my soaked from sweat body. It weather holds I will ride my bike right after church tonight. Wow! I enjoyed the singing at church this morning. Sandra Stone was cranking on all eight cylinders this morning and sat directly behind me and she sang so pretty. Her and James compliment each other. I sang too but I have such a poor voice that I try not to be heard too loudly.

Dear Sandra you have amazing talent and thank you for singing so beautifully. God told me to tell you so. (By the way, thank you James and Sandra for the tasty pears too! It is the little acts of kindness and self giving that build the foundation and make solid personal relationships.) A friend once said: "Glass cleaning is an art", which puts it all into perspective. Clean glass is important if only because dirt and other problems are more obvious on window glass. There are any number of glass cleaners available, most of which contain ammonia, that do a good job of cleaning glass. Home made recipes are popular and effective however. A simple one consists of 1/3 white vinegar and 2/3 water, with some people adding a teaspoon of car wash detergent. A more complicated concoction is: 1/2 cup sudsy ammonia 1 pint 70% isopropyl alcohol 1 tablespoon dish washing detergent 1 gallon water Others insist plain water is best, and some go to the trouble to use distilled water or collected rain water.

Lexington, North Carolina Coleslaw Recipe

This is a traditional Lexington, North Carolina coleslaw recipe. It combines cabbage, cider vinegar, and ketchup to make a flavorful slaw that goes perfectly with pork or chicken.

INGREDIENTS:
2 1/2 to 3 pounds white cabbage
1 cup cider vinegar
2/3 cup ketchup
1/2 cup water
3 tablespoons sugar
1 teaspoon salt
1/4 teaspoon cayenne pepper (optional)
1/4 teaspoon black pepper
1/4 teaspoon chile powder
dash of hot pepper sauce like Tobasco

PREPARATION:
Chopped cabbage. In a small saucepan disolve the sugar in the cider vinegar over a medium heat. Add other ingredients except the cabbage and mix well while simmering for about 10 minutes. Remove from heat before the mixture gets thick.

Indian (Canadian) Cough Syrup

This is an old Indian(canadian) remedy that I have used for 30 years and my mother used it longer than that. Buy a bunch of fresh beets. Cut them in half and place in a large shallow bowl. Sprinkle one half of a bag of brown sugar (2 lb.) over the beets and let sit overnight covered with plastic wrap. In the morning, drain off the juice (syrup) and then cover them again with the other pound of brown sugar, cover and let sit over night. Pour off the syrup and discard the beets. It's acutally quite pleasant to taste and will knock a cough dead in it's tracks. We call it Kick A-- Cough syrup and it never fails. Keep Refrigerated and it will last and last forever. Use as needed.

Ingredients:

Several non-toxic leaves, such as ivy, lemon or rose1/2 c. Semi-sweet chocolate or vanilla flavored chips (for white leaves)1 tsp. Crisco vegetable shortening

Directions:

* Before you do anything, wash and dry the leaves thoroughly.

* Melt chocolate and shortening together in the microwave for about 1 minute, stir to melt chips completely.

* Using a small paintbrush (new or very, very clean), brush the melted chocolate onto the backs of the leaves evenly and place onto waxed paper.

* Chill in the fridge until firm.

* Gently peel off the leaves from the chocolate and keep refrigerated until ready to use.

The first to come to my mind is that there is more foot work involved in traditional research in a library. You need to go up and down aisles and checking the card catalog, then returning to your table to page through multiple volumes in search of one simple fact. Whereas on the internet you simply go to a search engine, key in your topic and hit search. Credible information can be difficult to find. However, on the internet you can be sure to get the most up to date information on your topic. Educational sites are sometimes updated daily with current information. Libraries, on the other hand tend to have dated resources that are sometimes decades old.

Just in Case Your Day Was Boring

We'll it seems that sex and politics always go together. I had just jumped out of the shower and was reading your email as I was drying off the water dripping down my body. Did I tell you I have very soft skin? After a shower I like to be stroked softly all over my body, that soothes me I'm just rambling right now. I had a headache and had to take vicodin because the regular stuff was not working. Now I'm all fuzzy brained. I've been debating on whether I want to jump you as soon as you get through the door or wait until we can talk. We could to both. No, I rather consentrate on the physical completely if it comes to that. I do desire you, and so does my body. There are memories that come up and my body reacts to them. Are you reading this at work? Sorry, maybe I'm not. Maybe I did this on purpose, just in case your day was boring.
